Re-fuelling rear dump trucks

What happened?

During the re-fuelling of a Caterpillar 797B Rear Dump Truck, a large quantity of diesel emitted from the fuel tank overflow line and ignited. It is possible that the ignition source was the hot turbocharger, positioned directly below the overflow line. In the subsequent fire, the truck was totally lost.

Recommendations

  • Review the effectiveness of re-fuelling procedures.
  • Inspect the positioning of fuel tank overflow lines.
