RSHQ issues Safety Alert following fatal incident at Queensland quarry

22 April 2026

The state’s mining and quarrying safety regulator has issued a Safety Alert following a fatal incident at a Queensland quarry.

Resources Safety and Health Queensland (RSHQ) is investigating after a forklift operator lost their life while working at a quarry on Monday, 20 April.

RSHQ Operations General Manager Donna Heelan said, while a detailed investigation into the incident was underway, initial information indicated the worker was crushed when the forklift he was operating rolled onto its side while traveling on a ramp.

“Our team is on site and are conducting a thorough investigation,” she said.

“The purpose of the Safety Alert is to provide industry with initial advice to review the mitigation and management of risks associated with forklift operation to keep all workers as safe as possible.”

For detailed learnings and safety advice following this incident, read the Safety Alert on RSHQ’s website.
